Option 1: If your insurance gives you OUT-OF-NETWORK coverage/benefits, you will pay your designated copay or co-insurance under your plan and we will submit the claims on your behalf.
Option 2: If your insurance does not give you out-of-network benefits, you will pay under the Self-Pay rates. We can issue you a superbill and you can get re-imbursed by your insurance company. It is the patient responsibility to submit the superbill to their insurance. We suggest calling your insurance ahead of scheduling to make sure they will accept a superbill for reimbursement for out-of-network services.
With self pay rates, you know your costs up front and can plan and manage your money better. You also have the potential for reimbursement later on if you submit superbills to your insurance company. Under self-pay, your physical therapist can deliver whatever treatments are most beneficial to you. With insurance companies paying the bill, you are likely responsible for co-pays or co-insurance payments and insurance decides what physical therapy treatments they will cover. This means, treatments not covered by insurance can lead to unexpected charges for you. This also means your physical therapist must perform treatments that your insurance company deems acceptable, which are not always the BEST forms of treatment for you or your condition.
At Restore, we strive for the best possible outcomes and achieve them via our one-on-one individualized treatment sessions. Most other clinics have physical therapists treating multiple patients at once and this leads to less than best practice and less than optimal outcomes. With one-to-one sessions, we can focus better on your goals, spend more time on manual therapies if needed and can more easily adjust form and give you feedback.
You can call and leave a message if your practitioner can not get to the phone. You will always get a response via your preferred method text, e-mail or call. We have the ability to send SMS text messages if you sign our SMS text message waiver. If you have sensitive patient information, you may download the HIPPA compliant Spruce Health app and send us messages, pictures of insurance cards, imaging etc. via the app.
We require that you call, email or text to cancel your appointment at least 24 hours before your appointment time so that no late fees are charged. This ensures that patients that are waiting to get seen can be seen in a timely manner. You MUST receive verification that your appointment was canceled in a timely manner to not incur the $30 Late Cancel fee. No Show fee will be issued if you fail to advise your physical therapist/Instructor you will not attend your appointment.
