How do I pay for services?

Insurance Option or Self-Pay (Credit Card/FSA)

There are two versions to submit payment. Choose which option works best for you below.

Physical Therapy INSURANCE OPTION

In-Network Insurance Accepted

Traditional Medicare
Tricare (Authorized Provider)
Carefirst Bluecross Blueshield/Blue Choice (Except Federal Employee Program members whose subscriber ID begins with "R")

Out-of-network Insurance Accepted

Including, but not limited to:

Non-traditional Medicare plans (Medicare Advantage, Humana Medicare, etc.)
Cigna
Aetna
Other insurances with out of network coverage

Call your insurance company or look at your insurance card to make sure you have Out-of-Network Coverage. We can run a benefit check for you if you are unsure.

Physical Therapy Self-Pay OPTION

Self-Pay Applies to:

individuals without insurance
individuals that are not In-Network with us and do not possess Out-of-Network coverage under their current insurance policy to cover their visits.

If you bought a package and have not used it, you will be eligible for a refund only within the first 7 days of purchase . All pricing is fixed, non-negotiable and non-transferrable. All sessions completed are NON-REFUNDABLE.

Direct Access

You may start physical therapy without a physician referral under direct access. You must be able to provide a medical referral/prescription for physical therapy after 30 days.

May book online at any time, but Must specify "Pilates" or "Fitness" at the time of booking. MUST CALL FOR PACKAGE PRICING TO BE APPLIED TO YOUR ACCOUNT. Our online software currently does not have the option to purchase packages online.
